How to use this questionnaire

You can choose which sections you would like to respond to. You do not need to complete every section of the questionnaire.

Before you begin, you must complete Part A – Personal details, as we need this information in order to accept your submission. Once you have completed Part A, you can move through the other sections in any order and select only those you wish to comment on.

Before you submit your response, please make sure you have completed the Representation page, which appears at the end of the questionnaire. This information is required for the examination process and your submission cannot be accepted without it.

Your responses will still be counted and reviewed even if you choose to comment on only one section.
