The London Borough of Hackney and the City of London Corporation would like to hear your views about pharmacy services in your area.
A pharmacy may also be known as a chemist. In this instance it refers to a place you would use to get a prescription, buy medicines or seek medical advice on topics such as smoking, healthy weight, etc. This does not include pharmacies in hospitals.
This survey will help local providers understand how local people like you use pharmacy services and what may be needed in the future. It will also inform a local review of pharmacy services (pharmaceutical needs assessment) that will be published in April next year.
Your views are important, so please take 5-10 minutes to complete this survey. Please respond by Friday, September 19th 2014.
