Future City and Hackney Wellbeing Network service resident consultation

Closes 11 Feb 2020

Opened 14 Jan 2020

Overview

The City and Hackney Public Health Team are in the process of designing the future Wellbeing Network, a service that provides  public mental health support to City and Hackney residents. We are really interested to hear from a wide range of residents about what you think is important for the new service, whether you have used the service or not.

Please could you complete the following short survey, which should take no more than 15 minutes, by  11th February 2020?

Please also pass the link on to anybody else that you know who may be able to provide useful insight.

The feedback you provide will be confidential and will only be accessed by the staff carrying out the survey. The results may be presented in summary form and example quotations used in reports, but we will not disclose identifiable data.  Participation in the survey is optional and you may stop at any time.

As a thank you for your time you have given us, you will be entered into a prize draw to win one of three £30 shopping vouchers.  So that we can contact you if you win we will ask for your name, phone number and email address. These details will not be used for any other purpose. However, if you would prefer to remain anonymous and not be entered you do not need to provide this information.

Have you or somebody you know experienced poor mental health?  Do you live in Hackney or the City?
